Where do I send the undergrad acceptance of offer form and proof of payment once completed?

The agent needs to advise the student that the details of the Consultants is at the bottom of the offer letter.

The applicant is made an offer.  They receive an SMS an email with a link to accept the offer online.  The applicant will upload the acceptance of offer form, receipt of payment of the acceptance of offer fee and the indemnity form.
